Does Caffeine Cause Anxiety? Coffee, Your Nervous System, and Your Gut

Yes — caffeine can produce anxiety symptoms, and it is listed by MedlinePlus among the effects of consuming too much. But "too much" is deeply personal: a 2017 review found anxiety can be produced across a range as wide as 200 to 2,000 mg per day, with genetics and preexisting anxiety making some people far more sensitive than others. And there is a second, quieter mechanism most articles miss: caffeine reliably disrupts sleep, and poor sleep independently degrades focus and mood — so caffeine can manufacture the jittery, foggy state you are drinking it to escape. Here is what is actually happening, plus what coffee does to your gut that has nothing to do with caffeine at all.

What does caffeine do to your body?

MedlinePlus (NIH/NLM) describes caffeine simply: it "stimulates your central nervous system, which can make you feel more awake and give you a boost of energy," and it "stimulates, or excites, the brain and nervous system." The timing matters. Per MedlinePlus, "within one hour of eating or drinking caffeine, it reaches its peak level in your blood. You may continue to feel the effects of caffeine for four to six hours."

Mechanistically, a 2017 comprehensive safety review in Frontiers in Psychiatry explains that caffeine "works by binding to adenosine receptors located in the central and peripheral nervous systems," blocking their normal function. Adenosine is part of how your body signals that it is time to wind down. Caffeine does not add energy — it blocks the brake.

Can caffeine cause anxiety?

MedlinePlus lists anxiety directly among the problems caused by consuming too much caffeine, alongside "restlessness and shakiness," insomnia, headaches, dizziness and a fast heart rate. A second MedlinePlus page on caffeine in the diet lists the same picture: "a fast heart rate," "anxiety," "difficulty sleeping," "nausea and vomiting," "restlessness," and "tremors."

What the 2017 Frontiers in Psychiatry review adds is why the threshold varies so much between people. It reports that "anxiety can be produced at a wide range of doses (200–2,000 mg of caffeine/day)," particularly in people with preexisting anxiety disorders. It also identifies a genetic component: "common variants of the gene encoding for the A2a receptor can disrupt sleep or cause anxiety in some individuals after ingesting caffeine."

That is the crucial nuance. If two people drink identical coffee and one is fine while the other is wired and uneasy, that is not weakness or imagination — receptor genetics and baseline anxiety genuinely change the response.

How much caffeine is too much?

MedlinePlus states that "for most people, it is not harmful to consume up to 400 mg of caffeine a day," roughly "four 8 ounce cups (1 liter) of brewed or drip coffee." But cup size and strength swing the number: MedlinePlus puts a typical 8-ounce coffee at 95–200 mg and energy drinks at 70–100 mg per 8 ounces. Two large coffees can quietly reach the ceiling. MedlinePlus also warns that consuming very large amounts — over 1,200 mg — within a short period may lead to toxic effects such as seizures, and recommends limiting intake to less than 200 mg per day during pregnancy.

Stopping abruptly has its own cost. MedlinePlus lists caffeine withdrawal symptoms including drowsiness, headaches, irritability, and nausea and vomiting — which is a good argument for tapering rather than quitting cold if you are trying to find your threshold.

The sleep loop: caffeine's underrated anxiety route

The 2017 review is unambiguous: "Caffeine disrupts sleep in all populations studied." It further notes caffeine may "change the circadian rhythm," and describes the trap directly — caffeine causes sleep problems, which lead people to consume more caffeine for daytime alertness.

Now layer in what sleep loss does on its own. The National Heart, Lung, and Blood Institute (NIH) notes that with sleep deficiency "you might have trouble learning, focusing, and reacting," that it becomes "hard to judge other people's emotions and reactions," and that people may "feel frustrated, cranky, or worried in social situations."

Read those together and a pattern emerges: caffeine late in the day costs you sleep, and lost sleep produces exactly the unfocused, irritable, on-edge state that feels like anxiety. Before concluding that coffee itself makes you anxious, it is worth testing whether the real culprit is coffee's timing.

What coffee does to your gut — and why it is not the caffeine

This is the part most caffeine articles skip, and it is the most interesting.

Coffee moves your colon

A classic 1990 study in Gut surveyed 99 healthy volunteers and found 29% said coffee induced a desire to defecate. The researchers then ran manometry on 14 subjects and found an increase in rectosigmoid motility index within four minutes of drinking coffee, lasting at least 30 minutes — in eight "responders," with six subjects showing no response at all. The finding that matters: the effect occurred with both regular and decaffeinated coffee, while "there was no increase in the motility index in any subject after a drink of hot water." Coffee's effect on the colon is not a caffeine effect, and not everyone has it.

Coffee shapes your microbiome

A 2024 study in Nature Microbiology analysed 22,867 participants with detailed dietary data, integrated with public metagenomic data totalling 54,198 samples, and found coffee consumption is strongly and reproducibly associated with the gut bacterium Lawsonibacter asaccharolyticus. Median abundance of the species "ranged from 4.5- to 8-fold higher in the high compared with the never group." Again, this held for decaffeinated coffee too — the association is not caffeine-dependent, and the authors point to coffee polyphenol metabolites, including quinic acid and its derivatives, as candidate drivers.

So coffee is doing at least three separate things: caffeine acts on your nervous system, non-caffeine compounds act on your gut motility, and coffee's polyphenols act on your microbial community. Lumping all of that under "caffeine" is why people get confused about their own reactions.

Practical takeaways

  • Move it earlier before you cut it. Effects last four to six hours per MedlinePlus, so an afternoon coffee is still active at bedtime. Timing is often the whole fix.
  • Count your actual milligrams, not cups. At 95–200 mg per 8 ounces, a large speciality coffee can be two "cups" of the 400 mg guidance.
  • Taper, do not quit cold. Withdrawal headaches and irritability are documented and will muddy your read on whether caffeine was the problem.
  • Try decaf as an experiment. Because both the colonic and microbiome effects persist without caffeine, decaf is a genuine way to separate "caffeine bothers me" from "coffee bothers me."
  • Know when it is not the coffee. NIMH notes that anxiety disorders "involve more than occasional worry or fear" — anxiety "does not go away, is felt in many situations, and can get worse over time," and symptoms "can interfere with daily life and routine activities."
