Short answer: nausea and diarrhea are the two most common side effects of GLP-1 medicines, and they're worst when the dose goes up. In the Wegovy trials, 44% of adults had nausea (vs 16% on placebo) and 30% had diarrhea (vs 16%).[1] What helps most: smaller, slower, lower-fat meals, stopping at full, ginger or mint for queasiness, and serious attention to fluids. If you can't keep liquids down, call your prescriber.
Queasy after three bites, or running for the bathroom the day after your shot, is a rough way to start a medicine that's supposed to make life easier. Here's what's behind it and what actually helps.
Why does a GLP-1 cause nausea and diarrhea?
Nausea tracks with the stomach slowing down. Semaglutide delays gastric emptying,[1] so food stays in the stomach longer and it's easy to overfill it. A meal that was normal last month can now be too much.
Diarrhea is less well explained, and it's common: 30% on Wegovy vs 16% on placebo.[1] What's clear is the timing. GI reactions increased during dose escalation,[1] and in the main tirzepatide (Mounjaro, Zepbound) obesity trial, most GI side effects were mild to moderate and happened mainly during dose escalation.[2]
What helps nausea on a GLP-1?
A 2022 multidisciplinary expert consensus on GLP-1 side effects recommends:[3]
- Eat slowly and eat less. Smaller portions, only when you're really hungry, and stop at the first sign of full.
- Keep it low-fat and easy to digest. Greasy meals sit longest.
- Try crackers, apples, mint, or ginger (ginger root or ginger drinks).
- Avoid strong smells.
- Don't lie down after a meal, and don't eat close to bedtime.
- Get some fresh air and light activity.
- If you're vomiting, be extra careful with fluids and eat smaller amounts more often.
On ginger: the best human evidence is in pregnancy-related nausea, where a meta-analysis found it helped more than placebo.[4] It hasn't been studied in GLP-1 users, but it's a reasonable, low-risk thing to try. Check with your prescriber if you take blood thinners.

What helps diarrhea on a GLP-1?
From the same consensus:[3]
- Drink generously. Water first. The consensus also mentions water with lemon and a pinch of baking soda. An oral rehydration drink works too.
- Pause the usual triggers for a while: dairy, coffee, alcohol, soft drinks, and sweeteners ending in "-ol" (sorbitol, xylitol, maltitol).
- Ease off rough, high-fiber foods for now, like bran cereals, nuts, seeds, beans and artichokes.
Not all fiber acts the same, though. Psyllium is a soluble, gel-forming fiber that can firm loose stool as well as soften hard stool.[5] If you want to try it, start small and ask your prescriber or pharmacist first, and keep it 2 hours away from other pills.
Don't let dehydration sneak up on you
This is the one to take seriously. The Wegovy prescribing information warns that nausea, vomiting or diarrhea can cause fluid loss that leads to kidney problems.[1] Call your prescriber if you notice any of these:
- You can't keep fluids down
- Dizziness, dark urine, or barely peeing
- Diarrhea that lasts more than a couple of days, or any blood in your stool
- Severe stomach pain that won't go away, with or without vomiting
- Fever
Don't stop or change your dose on your own. Your prescriber may slow your dose schedule, which is one of the main levers they have.
